Point-of-service meal counts by child and meal type for a full week, with the daily totals row your sponsor asks for on reimbursement claims.
Record counts at the table as food is served. Counts written after the fact get disallowed.

One row per child with vaccine dates, exemption notes, next-due dates, and a physical exam column. Conditional formatting flags anything past due.

Will these forms satisfy my state licensing requirements?
They cover the fields most states ask for, but licensing rules differ by state and change. Check each form against your current licensing handbook before you rely on it, and write in your state's specific ratios and retention periods.
Can I use them if I run a home daycare?
Yes. The sign-in sheet, incident report, and immunization tracker are sized for small groups. Home providers licensed for six or fewer children use the same forms.
